Rebate Instructions

To redeem the student rebate and membership, you must:

  1. Cut out the membership application form from the back of your new White Book.
  2. Complete the form per the instructions.
  3. Attach to your completed application
    • A copy of your current class schedule or a letter from the Registrar stating that you are a full-time student
    • A copy of your original receipt for the White Book
  4. Return application to AIHA, P.O. Box 1519 Merrifield, VA 22116-9990 or fax to (703) 207-8673.

Please allow 3-4 weeks for processing your rebate and membership.

Rebate Limitations and Restrictions

The following limitations apply to the rebate and membership:

  • If a student does not meet the eligibility requirements for AIHA student membership, then the individual is not eligible for the rebate or the free membership. In order to qualify for student membership you must meet the following requirements:
  • A full time student at the college undergraduate or graduate level may become a student member upon application and yearly submission of a statement from his/her faculty advisor confirming status as a full-time student. Additionally, an undergraduate student taking at least one-half the credit hours required for full-time undergraduate student status may also become a student member upon application and yearly submission of a statement from his/her faculty advisor confirming the number of credit hours. Student members who qualify as part-time students may continue as student members for a maximum of eight years provided the required documentation is submitted annually. A student member may not vote or serve on the Board of Directors, but may serve as a non-voting member of a committee of AIHA.
  • If a student is a full, associate, or affiliate member of AIHA, he/she is not eligible for the rebate or the free student membership.
  • If the rebate and membership form is submitted September 1 or later, the student will receive membership for the remainder of the year PLUS the entire following year (16 months maximum).  If the rebate and membership form is submitted prior to September 1, then the student will receive membership for the remainder of the current calendar year.
  • If a student is already a student member of AIHA, he/she cannot receive an extension on his/her membership nor can he/she receive an additional refund for the price of the AIHA student membership fee. The student is still eligible for and shall receive the $70 rebate upon submission of the rebate request form.
  • If a student purchases the book directly from AIHA or any web-based bookstore (Amazon), the student is ineligible for the rebate.
  • This rebate cannot be combined with any other offer(s) and must be redeemed within six (6) months from the date of purchase.
  • This offer is valid for new book purchases ONLY.
  • Requests for consideration of changes to this policy are to be directed to the attention of AIHA’s product development manager, whose decision is final.

PLEASE NOTE: If you are ineligible for the rebate (i.e. purchased from AIHA or web-based bookstore) you will still receive the free membership as long as you meet the requirements of student membership. Please attach a copy of your current class schedule or a letter from registrar’s office confirming your status and a copy of your receipt for the White Book.
