Laboratory Accreditation Policies

2008 Policy Module Changes are now available. Changes have been made to the following modules: 2A, 2B, 2C, 2D, 3, 4, and 9. Changes went into effect on May 1, 2008.

Applications/Checklists/Guidelines/Forms

The 2008 Site Assessor Checklist is available.

The LQAP is committed to making the Site Assessor Checklist available to its accredited laboratories or other parties who already legally own a copy of the ISO 17025:2005 standard. To obtain a PDF version of the Site Assessor Checklist, you will need to send us proof of purchase of the ISO/IEC 17025:2005 standard (either a receipt or a signed affidavit, which can be found in the on-line Accreditation Application).

2008 Fee Schedule 

The LQAP fee schedule consists of the Laboratory Accreditation Programs Enrollment forms and fees, the Proficiency Analytical Testing (PAT) Programs Enrollment forms and fees, the Asbestos Analysts Registry (AAR) Enrollment form and fees and the Stock and Retest sample order forms.
