Notes:
Several departments had been identified to participate. The four departments with the highest incident rate for injuries and illnesses were identified. These departments were discussed at an Environment, Safety and Health Coordinating Council (ES&HCC) meeting. One purpose of the Council is to formulate and recommend to the Laboratory Director policies and procedures with regard to Environment, Safety and Health. From the information presented at the meeting, the Associate Laboratory Directors agreed that three departments should participate in a pilot study. Due to budgetary constraints, three out of four of those departments were randomly chosen to participate in the pilot study. These departments are from three separate divisions at SLAC. The departments are Operational Health Physics, Facilities and Plant Engineering / Maintenance.
Please note that the Facilities and Plant Engineering / Maintenance departments were merged into a new department called Site Engineering and Maintenance on January 1, 2000.
