Notes:
One method to help patients who may not qualify for a home visit, is to
conduct a brief home-survey in the clinic. The survey is
completed by the patient’s parent, entered into our
Database and will be used in the management of the patients disease
and as a tool for further study in trend found in patients who suffer from
a host of symptoms. We have over 1000 surveys completed since
last year within our general population.
