Important Course Return Information and Forms 

Requested forms for eLearning and Home Study Courses

Below are commonly requested forms for participants. The list excludes virtual event transfers and extensions as they do not apply to virtual events. If you have questions, please contact the distance learning assistant  or call (703) 849-8888. 

  

 Transfer Request Form
Students who enroll in a course may transfer the rights of that course over to someone else.

 Course Extension Form Students may extend their deadlines up to one year after the original expiration date.

 Course Evaluation Form
Evaluations must be submitted along with course requirements to earn credit for distance learning courses.

 Course Order Form                             
Order a eLearning archive, DVD, Video, or Self-study course by fax or email. 

 TeleWeb Topic Submission Form  Fill out and submit a submission form to AIHA Education for review.

Course Participant and Ordering Information

  • Before registering review all Distance Learning frequently asked questions at www.aiha.org/DLFAQs.  
  •  Member pricing on all Distance Learning courses is not available to organizational members.
  • Prepayment is required for all orders.
    Allow 3–4 weeks for delivery and/or lesson/test processing.
  • Prices are subject to change without notification.

Returns 

Return Policy excludes live virtual events and archives. The following pertains to course items and materials mailed to participants

It is AIHA’s policy not to sell or accept returns of partial products. Only unused, sealed, or shrink-wrapped products may be returned. If a product arrives damaged, return all course materials with a copy of the original invoice and an explanation for the return. Claims for incorrect or damaged shipments must be made within 30 calendar days of the ship date and be accompanied by a copy of the invoice.

Return Information and Fees

  • A $50 restocking fee will be applied for *products received within 30 calendar days of shipping.
  • A $135 restocking fee will be applied for *products received between 31 and 45 calendar days of shipping.
  • *Products received after 45 calendar days are nonrefundable.
  • TeleWeb Virtual Seminars’ cancellation and refund policies are listed individually online.

*The  return policy is applicable to certain products only: Self-Study Workbook, DVD and Video and CD courses.
