As workplace safety and accident prevention methods have evolved, numerous paradigms have emerged around incident investigations. For example, ‘each accident must have a root cause analysis, and ‘supervisors are the best persons to lead investigations’. With new research findings related to the causal factors of serious injuries and fatalities, it is time to take another look at incident reporting and investigation processes. In this webinar, we will talk about the limitations of the current paradigms, and the need to shift our thinking around incident investigations.​

Prerequisites

Experience in conducting root cause analysis investigations and knowledge of critical safety systems such as lockout/tag out, confined space entry, work at heights.​

  • Identify accident investigation paradigms that may exist in your organization.
  • Understand the foundational basis for these paradigms and why they create problems in accident investigation effectiveness.
  • Introduce new ways of thinking and acting to improve the overall effectiveness of accident investigation processes.

Presenters

Don Martin, MPH, CIH, CSP is Vice-President, Director of Consulting for BST, and he supports many safety improvement projects for clients in all industry sectors throughout the world. Don works with senior executives and executive management teams to develop and implement strategic solutions in safety leadership, culture change, and employee engagement. For the past 4 years, he has been leading BST’s global research project into the causes and prevention of serious injuries and fatalities. Don is a 36-year career safety and risk management professional. He is an accomplished executive coach and has provided personal development and safety leadership coaching to senior executives from the operational level to the boardroom level. Don also has extensive expertise in designing and implementing EHS management systems, risk management programs, and performance improvement initiatives for high-risk organizations. He is a Certified Industrial Hygienist and a Certified Safety Professional. Don received a Master of Public Health from the University of Texas and he also holds a B.S. in Environmental Science from Sam Houston State University.
