Hazards analysis can be intimidating: one mistake can literally be a life-or-death issue. What happens if a hazard is missed? What happens if a hazard is mistakenly characterized? What happens if a hazard is not adequately controlled? The instructors will present a unified method for the hazards identification, hazards screening, and hazards evaluation as well as control derivation and control hierarchy analysis. A thread analysis is a method to ensure identified hazards are analyzed and controlled. The hazards analysis process will be revealed in clear and concise discussion.

This PDC will be a discussion: attendees are encouraged to bring problems, experiences, and questions. Participants will be guided in a brainstorming activity to collectively identify hazards, brainstorm hazard events, assign consequences, and derive “credited” controls. Throughout the PDC, instructors will provide examples of the performance and value of a thread analysis to ensure a connection of the Hazard Analysis process from start to finish. Attendees will leave with the experience of the practical application of performing a hazards analysis with a connected Hazards Identification Checklist, Screening Evaluation, Hazards Analysis Table, and Qualitative Risk Analysis.

Learning Objectives

After this online PDC, participants will be able to:

  • Perform a hazards analysis based on a real world example.
  • Complete a hazard identification checklist on a real world example.
  • Use a risk matrix to develop a control hierarchy.
  • Complete a thread analysis on the hazards analysis results.
  • Compare their efforts/outcomes with a completed, reviewed, and approved analysis.

Presenters

Kelsey L Forde, CIH, CSP, CHMM

Ms. Kelsey Forde is Owner/President and Principal EHS Professional and System Safety Engineer for Parvati Consulting LLC & Parvati Government Services Inc. Kelsey has nearly 20-years of EHS consulting experience and holds a Master of Science degree in Environment, Health, and Safety and a Bachelor of Science degree in Cellular Biology from the University of Minnesota. Ms. Forde’s primary responsibilities and areas of expertise are centered around the identification of workplace hazards and development of consequence analysis associated with hazard analysis, safety assessments, primary hazard screens, readiness reviews, and compliance auditing techniques for a variety of clients including the Department of Energy (DOE), commercial, industrial, and private clients. Additionally, Kelsey was a notable major contributor to the update of DOE-HDBK-1163-2020, Integration of Hazard Analyses, and was corecipient of the 2022 Department of Energy (DOE), Energy Facilities Contractor Group (EFCOG) Teamwork Award for these efforts.

Timothy S Stirrup, CSP, CHMM, REM

Mr. Timothy Stirrup is a Partner and Principal EHS Professional and System Safety Engineer with Parvati Consulting LLC & Parvati Government Services Inc. Tim has over 35-years of experience as an EHS professional and holds a Bachelor of Science degree in Biology and Bachelor of Science degree Chemistry from the New Mexico Institute of Mining and Technology. Mr. Stirrup’s primary responsibilities and areas of expertise are centered around establishing the framework for clients in Hazard Analysis within a diverse set of industrial facilities including nuclear facilities, accelerators, semiconductor facilities, R&D laboratories, and dynamic energetic materials testing facilities. Mr.Stirrup provides direct input to Line/Executive Management and Regulators to ensure solutions for continued organizational success. Tim is respected for his ability to work with highly functional teams and solve diverse, complex problems.
